What to Bring

To help us make the most of your visit, please bring the following items to your appointment. If you don’t have everything, don’t worry — we’ll still be happy to see you.

The things that the patients should bring to the office are:

Photo ID

a valid driver’s license, state ID, passport, or other government-issued photo ID

Insurance Information

Please bring your current medical and/or vision insurance cards. If you’re covered under someone else’s insurance, bring their information as well

Current Glasses & Contact Lenses

Bring your current or most recent glasses, prescription sunglasses, and contact lenses. If possible, bring your contact lens boxes or packaging so we can verify the exact lens information

Previous Eye Care Records

If you’ve seen another eye doctor recently, bringing your previous records can help us better understand your eye health and history. We can also request records from your previous provider with your authorization

Medication & Medical Information

Bring a current list of your medications, allergies, and any significant medical conditions or surgeries

Don't Have Everything?

Don’t worry if you don’t have every item on the list. Our team can help you obtain previous records or verify your insurance information. Just bring what you have, and we’ll take care of the rest
